RULES AND REQUIREMENTS FOR FAST TRACKS

To ensure the health and safety of your pet and of our other guests, we require that all our clients comply with the following rules and requirements:

  1. All dogs must successfully complete a full temperament assessment with Fast Tracks certified trainer prior to attending doggie daycare. I (the owner) understand that if my dog(s) has a history of aggression or biting, Fast Tracks reserve the right to refuse service. I have disclosed to Fast Tracks all known dangers associated with my dog(s).
  2. DAYS, HOURS, AND PROMPTNESS: Unscheduled appointments interfere with the interactive natural and security of our facility. Pickups, drop offs, and tours are scheduled by appointments only. Call to make a reservation. For the safety of our dogs and staff, we appreciate your promptness. Please do not be more than 10 minutes early or late for your appointment. If you need to adjust your time, please call!
  3. All dogs must have up-to-date vaccinations. Owners must submit proof that t heir dogs have current DHLPP (distemper), rabies and bordetella (kennel cough) vaccinations.
  4. Vaccinations must be given at least 48 hours prior to attending Fast Tracks. (7-14 days recommended). If your dog is under doctor’s orders not to be re-vaccinated, please supply a note from your vet. Follow your veterinarian’s recommendation(s); they may differ from our requirements. It is YOUR responsibility to provide on-going verification of current vaccinations.
  5. All dogs must be in good health. Owners will certify that their dog (s) are in good health and have not been ill with a communicable condition in the last 30 days. Upon admission, all dogs must be free from any condition that could potentially jeopardize other guests, including fleas or ticks. Dogs that have been ill with a communicable condition with the last 30 days will require veterinary certification of health to be admitted or readmitted.
  6. All dogs must have a complete, up-to-date and approved application on file prior to attending FastTracks.
    Please, do not bring personal items like bones or toys. We have all the food/water bowls, bedding, measuring cups, treats, and toys necessary for your dog's care.
  7. If a pet that is present for daycare if found to have fleas and or ticks, it will be bathed and medically treated at the owners expense.
  8. If any medical problems develop while my dog(s) is in the care of Fast Tracks, I authorize for the Veterinarian above listed by you to do whatever they deem necessary for the safety, health and well being of my dog(s). Furthermore, I agree to assume full responsibility for any and all expenses incurred. I understand that all medical or grooming services are charged separately from doggie daycare fees.
  9. Vaccinations must be given at least 48 hours prior to attending Fast Tracks. (7-14 days recommended).   10. All Food will be brought and provided by me the owner for overnight boarding. Failure to provide food may result in additional charges.
  11. I understand that any personal items I bring with my dog may get lost or destroyed and will not hold Fast Tracks responsible for replacement. This includes, but is not limited to, dog beds, dog toys, blankets, bowls, collars, leashes, food, etc.
  12. LEASHES: All dogs must be restrained by color & leash during arrival, departure, and at all times while on Fast Tracks Property. NO FLEXI LEASH!Do not let your dog run loose or approach fences or other dogs/clients while on our property or in our lobby.
  13. CRATE OPTION: Due to high levels of boarding during the holidays we will offer crate options to CURRENT clients if all other kennels are full.
  14. MEDICATIONS: We are happy to give medications to your dog(s) at no extra charge, however if a dog needs injections, special treatment, carried in/outdoors, up/down stairs, a VIP handling fee will apply. All medications must be labeled with name and instructions.
  15. PICTURES-VIDEOS: The owner gives permission to Fast Tracks to use pictures taken during the training classes, dog daycare or boarding on marketing material, advertising and social networking environments.
  16. I agree if I bring my dog to Fast Tracks that I will pick my dog up by closing time. If I am not able to pick up my dog by closing time, that I will contact Fast Tracks to arrange another pick up time during regular business hours.
  17. A late pick up fee of $5.00 per 15 minutes up until 6:00 PM. If you are not here to pick up your dog(s) by 6:00 PM, they will be put up for boarding and you will have to pick up the next day during business hours. You will be responsible to pay the daycare fee plus boarding charge at pick up.
  18. An additional late charge of $25.00 if you schedule to pick up/drop off after hours.
  19. PACKAGES: Owner understands that pre-paid packages are NONREFUNDABLE and do have expiration dates.

HOLD HARMLESS AGREEMENT FOR FAST TRACKS

As a condition of using our serviced for your pets, the following Waiver & Assumption to Hold Harmless must be signed:

By choosing to utilize the services, participate in the activities, and train my pet(s) at Fast Tracks I agree to the following:

  1. I agree to pay the rates that are in effect at the time my pet is at Fast Tracks. I am aware that extra charges may be incurred and I agree to pay them at the time of pick-up. Examples include but are not limited to the following, grooming, emergency or medical treatment. There is a $20.00 fee for all returned checks.
  2. I understand that an interactive play setting is not without some risk of injury. I understand that despite all of the daycare participants being temperament-tested prior to entering dog daycare, boarding and training appearing healthy, and being handled with the greatest amount of care an foresight, dogs are not\always predictable and the unexpected may occur. I (the pet parent) recognize that the benefits of an interactive play group are invaluable to my dog and accept the potential risks. I further agree to pay veterinary/medical expensed incurred as a result of injury to caused by my dog(s).
  3. If my pet appears to be ill, I authorize Fast Tracks to engage the services of the veterinarian listed above by you, at my expense, to give other required attention, and to make whatever decisions are required for my pet's treatment. I agree to pay all veterinary charges incurred by my pet. I understand that all attempts will be made to contact me regarding my pet. In the event I cannot be reached, I will not hold Fast Tracks liable for failure to seek services or decisions made under this contract.
  4. I understand Fast Tracks will exercise all due diligence and care in the guardianship of my pet during my absence. I hereby waive and release Fast Tracks, its employees, owners, and agents from any and all liability of any nature, for injure and damage, including that which may result from the action of any dog including my own. Furthermore, I expressly assume the risk of such damage or injury while my dog participates in or attends any function of Fast Tracks doggie daycare, while on the grounds or the surrounding area there to.
  5. DAMAGES: It is the responsibly of the pet owner for any and all damages done to our facilities and/or furnishings. You will be billed for damages when you pick up your pet.
  6. I understand that I may be required to provide Fast Tracks documented vaccination records and to keep the vaccination up-to-date for the duration of my dog's participation at doggie daycare.
  7. I agree to abide by Fast Tracks vaccinations and health requirements. This includes maintaining current Rabies, Distemper, Bordetella vaccinations.
  8. Abandonment of Animal: I understand that, in accordance with the Iowa State Law, a dog not claimed after close of business on the day s/he attended Fast Tracks for daycare, boarding, or training, by the agreed upon pick up date and time, is considered abandoned. At which time Fast Tracks will send a notice by certified mail to the last known address of the owner. For fourteen (14) days after mailing the notice the owner has the right to reclaim their dogupon payment of the dog’s entire stay and all services rendered at the listed price of Fast Tracks. After 14 days, your dog becomes property of Fast Tracks and can be taken to a shelter or re-homed to the home Fast Tracks choosing. (Chapter 162.19 of the Iowa Code)
  9. If a dog dies at the facility, Fast Tracks has the right to require a necropsy (dog autopsy) at Fast Tracks expense.
  10. On behalf of myself and any and all other owners of this pet, I have read and agree to the terms of this contract. I warrant that I have the authority to represent any and all other owners of this pet in signing the contract.

Additional Information:

Health:

  1. Please do not bring your dog to Fast Tracks when they are sick.
  2. If your dog is acting at all “off” please keep him or her home.
  3. If your dog is showing any of the following symptoms: vomiting, diarrhea, runny nose, goopy eyes, coughing or hacking please do not bring them to Fast Tracks.
  4. Any dog that arrives at Fast Tracks that we suspect may be sick will be sent home.
  5. If your dog has been sick, please do not bring them back to Fast Tracks until they have been SYMPTOM FREE for more than 5 days.

Daycare Schedule:

  1. 7am – 9am: Drop Off (dogs may get kenneled for safety)
  2. 9am -11am: Playtime indoors and outdoors
  3. 11am-1pm: Lunch/Nap time (Dogs kenneled)
  4. 1pm-4:30pm: Playtime indoors and outdoors
  5. 430pm-5:30pm: Pick up (Dogs kenneled for safety)

Holidays:

  1. Daycare: Fast Tracks has daycare restrictions surrounding holidays. Before and after most major holidays, and during the holiday, including Spring Break, we only take very limited number of daycare dogs or sometimes this means daycare is CLOSED.
  2. Boarding: There is a 3 (three) night minimum stay required for boarding over thanksgiving and Christmas.
  3. We are closed on major Holidays.
  4. Deposits: We require 50% deposit for boarding over Thanksgiving and Christmas. If you cancel a week before, you will get a refund. If you cancel the week of, you will not get a refund.
